NON-SURGERY AESTHETIC PROCEDURES
FACE LIFT WITH THREADS
''FACE LIFT- V-LIFT'-FACE LIFT''
"NON-SURGICAL EYEBROW AESTHETICS"
"NON-SURGICALNOSE AESTHETICS"
In our world, where change and transformation are inevitable, socio-cultural values are also changing day by day, and the demands of both male and female patients for dermocosmetic procedures are increasing. Expectations from aesthetic applications also differ in this process. Expectations of patients in all aesthetic and dermocosmetic procedures today; Applications can be done quickly even during coffee breaks or lunch breaks, long-lasting effects, reliable and modern, quick return to work and daily life, that is, social life after the application, and having as few side effects as possible. For this purpose, the methods defined as “Non-Surgical Aesthetic Procedures”, which are defined as general anesthesia, operating room conditions, rapid recovery and results without hospitalization, have become more frequently asked in recent years.
With aging, sagging occurs in almost every part of the body, especially in the face, neck, arms and hip regions, due to the decrease in the support tissue between the skin and the muscles, the occurrence of abrasions in the bone structures and the effect of gravity. The most common examples are the drooping of the eyebrows, the presence of bags under the eyes, the decrease in the fullness of the cheeks and the sagging of the jaw line, the accumulation of a second chin under the chin (also called the jaw line - bulldog chin), the sagging of the breasts, hips and neck skin.
We see the historical dimension of facelift in the gold wires on the face of Cleopatra, which was found as a result of archaeological studies in Egypt. As a matter of fact, even centuries ago, human beings gave importance to their youthful and aesthetic appearance. With the technology that has been changing and developing since then, this type of procedure has been carried out in a much shorter time and in a much more comfortable way.
What is a facelift with threads?
When it comes to face lifting with threads, we see that these threads (fusible and non-melting) have been used for a long time in general and aesthetic surgery to hang the tissues upwards. The special threads used in the facelift - hanging method with threads are actually the same as the threads used in surgery, but it is extremely important that they are made under local anesthesia without cutting and sewing the skin, without requiring hospitalization conditions in the operating room and hospital (necessarily under sterilization conditions). On the other hand, the fact that the patient's consciousness is open, that he can see the application and its results instantly with a mirror, and that the patient can participate in the evaluation with the doctor at every stage of the application provides a much more special situation.
Our aim in face lift with threads is to pull the facial or body tissues upwards to their normal anatomical areas. The application is based on placing the threads under the skin with special needles. Local anesthesia, sometimes with creams and sometimes with needles at the application points (dental needles - needles used in dentistry), simplifies the facelift procedure and increases comfort.
Can a facelift with threads be applied in other treatments?
Yes, during or after the application of the method, other applications such as laser, lipolysis (fat melting), botox and filling, if necessary, provide a great advantage.
For which age is the thread lift procedure suitable?
Thread lift-suspension method is generally ideal for people aged 30-60 years who do not have very dense tissue sagging and deep facial lines.
Do all sagging benefit from thread lift?
In fact, age is not a criterion for application, but the degree of sagging of the skin and soft tissues and their suitability for hanging are important. Very good results can be obtained in a short time for people who have slight facial sagging, tired expressions, and those who want to look younger and more dynamic and rested. It is more acceptable to the patient and his social environment, as it does not create a "tense, tense" facial expression as in classical facelift surgeries. However, although the thread lift-suspension method seems to be simple and easy to apply, it is not an alternative to face lift operations. The aesthetic results to be obtained as a result of this method are quite inadequate, especially in patients who need a serious operation such as a facelift with aesthetic surgery. Therefore, classical facelift operations are still the most ideal (gold standard) choice for patients with significant tissue losses, collapses and skin sagging on the face.
How long is the application period?
The application time is 30-45 minutes, even for the entire face. For this reason, it can be applied even during lunch breaks or weekends.
What are the side effects of thread facelift?
The side effects of the facelift-suspension method with threads are very few. Almost every patient may have mild pain, mild edema that may last for several days, and rarely minor bruises. In addition, infection may occur in people who do not pay attention to cleaning and protection. Although allergies can develop against threads that are used very rarely, we have never encountered such a side effect until today. If it is not performed in expert hands (this is very important), side effects such as vascular and nerve damage, skin collapses and asymmetry may occur against application errors.
How long does the effectiveness of face lift treatment with threads last?
Activity duration; It is the disappearance of the clinical response over time and means that the stretched tissues return to their original state. Especially in areas where dynamic movements such as forehead and neck are high, aesthetic problems can be restored more quickly, so it should be used in combination with botox or other methods in these areas. In other facial areas, the average permanence period is 12-60 months. This period varies depending on the type of threadused and personal factors.
Will there be a worse sagging after a face lift with threads?
No, a worse hiding does not occur afterward, but because people get used to their new looks and forget their old ways, they may feel as if I am worse than my old self when the effectiveness is lost. Therefore, touch-ups (both with thread or other methods) before the event is completely lost will help to eliminate this feeling.
What should be considered after the facelift with threads?
After the application, it is possible to return to daily activities immediately. Washing the face is required after 1 day. In the first 7-10 days, the mouth should be opened very wide (especially dental examinations and treatments should be postponed during this period), exaggerated facial expressions and hard foods should be avoided. It is requested to stay away from heavy exercises for 5 days, to stay away from sudden temperature increase environments (hot steam bath, sauna and solarium, etc.), not to use cosmetics for the first 12 hours after the application. Again, sun rays and solarium are prohibited for 2 weeks. Rubbing the face (massage, peeling and laser epilation) and lying face down should be avoided until 3 weeks after the procedure. It is also important not to put too much pressure on the face. At the end of these 3 weeks, the treated areas can be started to move normally. It is natural to have some swelling, redness or bruises in the first few days after the procedure. Ice compresses can be applied to reduce these. The feeling of discomfort on the face of the patient is very low compared to other surgical operations, but as it can be seen in every subcutaneous procedure, a slight itching sensation, burning and stinging may be seen in the first few days after the procedure. Painkillers can be used if any pain is felt. On the first day after the procedure, keeping the head elevated with a few pillows, especially while sleeping, helps to reduce swelling (edema). Oral and smear antibiotic therapy is given for 5-7 days to prevent possible infection.
When will the effectiveness of the thread lift application become clear?
Although it is partially noticeable immediately after the procedure, the completed real result appears after a few weeks (between 3 weeks and 3 months). The natural (skin's own collagen) collagen formation in the skin tissue continues to create a lifting effect that will continue for the first six months with a gradually increasing lifting-hanging effect by wrapping around the threads, and after that, an average of 18 months of effectiveness is achieved. Again, these periods may be extended depending on the type of thread used.
What threads are used?
Threads used for stretching, wrapping and suspending are very diverse. What creates this difference, when applied to the tissue, changes according to their permanence in the body or their physical structure.
According to the residence time in the tissue;
Permanent threads; they can remain for life without causing a reaction in the tissue. Threads, which have been used in surgeries since ancient times and are described as internal sutures among the public, are permanent threads. For example, prolene threads used in heart surgeries or APTOS threads used for suspension-stretching are in this group.
temporary threads; these, on the other hand, have the characteristics of disintegrating and disappearing in the tissue after a while. Their duration can vary from 6 months to 1 year. Thread lift, Elastic suture, Silhouette soft threads are in this group.
According to the structure of the threads;
flat threads; they have a smooth surface like conventional sutures. They are still used for suspending tissues under the skin in surgical facelift operations.
Notched-Hooked-Awned threads; On their surface, there are knot-shaped embossments or hooks in the threadstructure. Those with ridges are defined as non-aggressive notched threads, and those with hooks are defined as aggressive notched threads.
Hooked threads are divided into two as unidirectional and bidirectional according to the directions of the hooks on the thread.
Thread lift or V lift application
It is also defined as “Thread lift”, “Meso threads”, “Lead Fine Lift”. It is also defined as V lift because it gives an aesthetic and youthful appearance to the face in the form of a V starting from the chin after the application.
The threads used contain PDO-Polydioxanone substance. After 2-3 months, the threads lose their tension effect by 50%, PDO completely turns into water and carbonic acid, and after 6-8 months it completely disappears from the tissue. The new support tissue formed by the biological effect of the tissue remains for 18-24 months. There are varieties of threads with single-straight (Monoflament), double or multiple spirals (Tornado), screw-shaped (Screw) and hooks on the outside (Cog) as in aptos threads. These threads are placed in special very fine needles. The threads are placed on the desired tissue with the help of these needles. There are different length forms of threads and needles.
In this method, it is aimed to tighten and lift the sagging areas on the face or body and to give the contours again. The most important characteristics of the threads used are that they dissolve completely over time and are completely eliminated from the body, and they are defined as biocompatible threads due to these properties. It can be applied to all lower layers of the skin, from the dermis (the lower layer of the skin) to the suprabone membrane.
What happens after the threads melt?
After the application of these threads with special fine needles under the skin, new support tissue (collagen fibers) is formed around the threads. This causes the effect to last longer even if the threads are lost.
Are there any other effects besides lifting?
With the new collagen formed, increase in skin tone and resistance, shrinkage of pores with the formation of new tissue, opening in skin color and a more homogeneous appearance, with the development of new vascular structures, antiaging effects are also seen in addition to lifting.
In face or body applications, it does not affect the blood circulation of the muscle when applied into the muscle, and if it is made along the muscle axes, it shows a botulinum toxin-like effect, softening and relaxing this muscle. If it is desired to strengthen the muscle and remove the tissues on it, the application is performed opposite to the muscle axis.
There are also studies showing that when applied into adipose tissue, it causes lipolysis.
Are the threads one type and is the application painful?
The lengths of needles and threads range from 24mm to 90mm. thread types are determined according to the application area or the shape and condition of the face. For example, while short and thin threads are preferred under the eyes, longer threads are preferred in the neck area. There is very little pain during the application. Local anesthetic (numbing) cream or needles (for hooked threads) and ice are applied beforehand.
Since hooked threads are thicker and longer, needle anesthesia is definitely preferred when used with this thread.
What are the application techniques?
The application can be in the form of zig-zags, a star, lattice or web (in some places the term spider web is also used) from the periphery to the middle. Again, one or more of these techniques can be used together, depending on the application area and the condition of the patient.
Can these threads be used for rhinoplasty, under-eye wrinkles or eyebrow lifting?
Yes, it is used with or without fillers and botox to lift and reshape the tip of the nose. Again, they can be preferred for tissue recovery, reduction in bagging and wrinkles under the eyes. Another area of use is eyebrow suspension, and since the forehead area is very mobile, it is usually applied together with botox.
Who is not a suitable candidate?
It cannot be used in patients with autoimmune disease, systemic disease, bleeding tendency such as hemophilia, cancer patients, skin infection or inflammation in the application area, those with the risk of keloid and scarring (impaired tissue healing), febrile diseases, pregnancy and breastfeeding periods.
Silhouette Soft threads-Youth Thread
(ropes with cones)
Silhoutte Soft suspension systems, which have been developed in recent years, are the most perfect with their features. With Silhouette Soft threads containing various numbers of small cones on them, a rejuvenated new face can be obtained without the need for general anesthesia, hospitalization and recovery period. They are produced from different materials than the threads mentioned above. Again, these threads are threads that completely dissolve in the skin. They consist of 2 basic structures. The main basic structure is a thread made of poly-L-lactic acid (PLLA). The second basic structure consists of transparent cones, which are made of poly-L-Lactic acid and Glycolic acid, and are completely soluble in the body.
Cones in Silhouette Soft threads are 2-sided. In this way, various techniques such as 'U' technique, Linear technique, V technique, Cross technique, etc., the sagging degree of the person is determined by the number of threads to be placed. 4+4=8 cone, 6+6=12 cone, 8+8=16 cone types are available. thread types and application techniques can be combined.
Threads made from these materials have been used as suture threads in surgeries for many years, just like PDO threads. When applied to the skin, they do not develop an allergic reaction, do not cause foreign body reaction, and disappear completely over time.
The advantages compared to PDO threads are that the lifting effects are much stronger, the effectiveness begins to be seen immediately and gradually increases. Again, 3-6 months should pass for full effectiveness. Fewer threads are sufficient.
Disadvantages compared to PDO threads are less general skin tone and increase in resistance, shrinkage of pores with new tissue formation, lighter skin color and a more homogeneous appearance.
The things to be considered before and after, to whom it can be applied and the side effects that can be seen are similar to PDO threads.
Depending on the patient's condition, these thread types can be used together or separately.
